Consent for Research Participation

  • I hereby consent to participate as a subject in a research project entitled "Instructional Systems Design 3" conducted by Dr. Scott W. Brown of the Department of Educational Psychology at the University of Connecticut. I can contact the project director by e-mail at sbrown@uconnvm.uconn.edu.
  • I understand that I will complete a survey with questions pertaining to my knowledge, attitudes and behaviors pertaining to instructional design. I also understand that I will be asked about my beliefs in my own abilities.
  • I understand that my participation is completely voluntary, and that I am free to withdraw from the study at any time I choose, without penalty. I understand that I must be at least 18 years old to participate in this survey.
  • I understand that this project is not expected to involve risks of harm any greater than those ordinarily encountered in daily life. I also understand that it is not possible to identify all potential risks in any procedure, but that all reasonable safeguards have been taken to minimize potential risks.
  • I understand that the results of this project will be coded in such a way that my identity will not be physically attached to the final data that I produce.
  • I understand that the results of this research may be published or reported to government agencies, funding agencies, or scientific groups, but that my name or any identifying tags will not be associated in any way with any published results.
